Makeup Artist Booking Form: What to Ask Before You Hold a Date

Written by Insha I., co-founder of Artisee October 2026 6 min read

A bride asks if you are free on the 14th. Before you say yes, you need about a dozen answers: what time everyone must be ready, how many faces, where you are going, and what you quoted. A booking form collects them once, in writing, so the date you hold is the job you will actually do.

Most booking trouble starts with a date held on a guess. You said yes to the 14th in a DM, then learned there are seven bridesmaids, the venue is an hour away, and everyone has to be ready by 9am. The price you quoted no longer fits the job.

A booking form fixes that by asking the questions before you commit. This guide covers what to ask, why each answer matters, and when to send it. A one-page form you can print or send is free at the end: download the PDF.

Booking form or consultation form?

They are easy to blur, and they do different jobs. The booking form is about the job: the date, the time, the address, the number of faces and the money. The consultation form is about the person: her skin, her allergies, the look she has in mind. Send the booking form when an enquiry turns serious, and a makeup consultation form closer to the trial or the day.

What to ask before you hold a date

Every question here changes either your price or your morning. If an answer would not change either, leave it off the form.

The ready-by time matters more than the ceremony time. It is the fixed point you plan backwards from, and building a bridal hair and makeup timeline starts from exactly that number.

"Hold the date on the answers, not on the first message."

Why each answer changes the job

Party size changes the price and the start time. Seven faces is not one bride plus a bit. At forty five minutes to an hour each, it decides whether you start at 5am or bring a second artist.

The address changes travel. Distance, parking and stairs with a full kit all cost time. Ask now and put a travel fee in the quote, rather than absorbing it on the day.

Allergies change your kit. A bridal party is several people you have never met. One line on the form tells you which products to leave out before you pack.

The money changes whether the date is held. Writing the quoted total, the deposit and the balance due on the form means you and the client agree on the numbers before anything is signed.

When to send it

Send it when the enquiry turns real

Once she has a date and wants a price, send the form instead of answering ten questions one DM at a time.

Quote from the answers

Price the actual party, travel and timing. The form makes the quote easy to explain, because every line came from her.

Send the contract

Turn the answers into terms: the date, the services, the deposit and the cancellation policy, signed before the day.

Hold the date when the deposit arrives

Until then it is an enquiry. Say so on the form, so nobody assumes the date is theirs because they asked first.

The form gathers the facts; the contract makes them binding. The bridal makeup contract template covers the trial, deposit and travel clauses that follow from these answers, and a makeup artist cancellation policy sets out what happens if the date moves.

How Artisee handles bookings for you

With Artisee, the booking link does much of this form’s work. A client sees the times you are actually free, worked out from your weekly hours, time off and buffers, and sends a request. It lands in your Pending requests inbox, and nothing is confirmed until you approve it.

Each booking then records the price, the deposit you have taken and the balance still due, and the service agreement fills in from the booking, ready to sign on her phone. The public booking link is part of the Pro plan, and every new account starts with a month of Pro.

Frequently asked questions

Yes. The PDF downloads instantly with no email address and no account. It is one printable page you can fill in by hand, print for a consultation, or send to a client as a file.
A booking form holds the date and the logistics, who, when, where, how many faces and what you quoted. A consultation form is about the person in the chair, her skin, allergies and the look she wants. You usually send the booking form first and the consultation form closer to the day.
The event date, the time everyone must be ready by, the getting-ready address, how many people need makeup and who they are, whether a trial is wanted, lashes, allergies in the party, parking and access, and the quoted price, travel fee and deposit.
No. The form gathers the facts, and the contract turns them into terms both of you sign, including the deposit and the cancellation policy. Use the form's answers to fill in the contract, then send the contract before you treat the date as held.
As soon as an enquiry looks real and before you quote, so your price reflects the actual party size, travel and timing. Ask for it back with the deposit, and hold the date only once both arrive.
